Do we take your Insurance?

Yes! We do accept most insurance plans. We are In-Network Providers with Blue Cross Blue Shield as well as United Healthcare. If we are Out-of-Network with your plan, we can still file your claims. We will verify your benefits before your visit, and help you compare the cost vs. using a self pay rate.

We accept HSA and FSA cards!

Baffled by premiums, deductibles and out-of-pocket maximums? Here is an overview of health insurance that will help clear things up and give you a better sense of how your money is spent.

Medicare

The short answer is, yes, we accept Medicare, Supplemental, and Medicare Advantage plans. If you are interested in using your Medicare coverage, bring your cards to your initial appointment and we will verify your deductible, coinsurance, and how many visits you have remaining. If you have any questions, please call our office at (817) 251-7250.

Here is a short article we wrote over insurance, medicare, and Chiropractic.

What are the “SeLf-pay”/Out-Of-Pocket rates?

There is a lot to be said about paying for your care by yourself. Not only are you taking your health into your own hands, but it also puts you and the doctor in the drivers seat. Many people are unaware when they use their insurance, much of their care is dictated by the insurance company and not the doctor at all. Self pay rates are often the most affordable option to pay in all doctor offices you attend.

What about self submitting insurance?

Yes, that is a great secondary option to not only pay self pay rates, but then submit your charges on your own to your insurance company to apply to your deductible and/or get your reimbursement directly. Let us know if this is an option you’re interested in.

Here are The 2024/25 self pay rates:

New Patient Exam (60 mins) $225

Office Visit $75

What about children? (check out this blog post)

We love when families see the benefit of Chiropractic on their children. If the issues the child is having is severe enough, they will be placed on a treatment plan. The good thing is, children respond to care very quickly and often become “wellness” or “maintenance” patients after a few visits.

Pediatric New Patient Exam $75

Pediatric Office Visits $35

financial hardship?

If you have any questions about rates, or find that you need care but are having a hard time affording it, please contact us at MoultonChiropractic@gmail.com and we will reach out to you to make sure you are taken care of.

Medicaid & TRICARE

Unfortunately we currently do not accept TRICARE insurance plans, but we do accept Medicare and Medicaid. If you are interested in using your Medicare policy, call our office prior to scheduling your appointment (817) 251-7250.

billing questions?

Please send all questions and inquires to Kris Moulton at MoultonChiropractic@gmail.com subject line “billing” and we can make sure you get help. You may also call him directly at (402) 452-7598.

Cancellation Policy:

All visits must be cancelled or rescheduled 24 hours in advance. Please contact us by phone, email, or text to let us know.

  • All no-shows or late cancellations for office visits are charged a fee of $30. Contact us for questions regarding cancellations.

A Note: We try to avoid late cancel fees as much as possible. Our preference is to reschedule the appointment. We understand life happens, but since we block 1-1 time with doctors we need to take steps to help prevent no call/no shows which take up appointment slots that someone may have needed. This is why we always encourage a reschedule over a cancellation of an appointment.
